Computerized National Identity Card. Must be currently valid (not expired).

  • Clear scan of both Front and Back sides
  • Ensure all text and photo are clearly visible

Latest Pay Slip

Your most recent official pay slip issued by the Accountant General / District Accounts Office.

  • Must clearly show your Name, BPS, and School
  • Not older than 2 months

Driving License

A valid motorcycle driving license issued by Punjab Traffic Police.

  • A valid Learner's Permit is also acceptable for the initial application

Passport Size Photo

A recent, professional passport-sized photograph.

  • Blue or white background
  • Clear visibility of the face
